Job summary

This role facilitates conversations between interdisciplinary care teams including providers and hospital systems to deliver high quality, cost-effective care coordination.

The ideal candidate is an RN, BSN, or MSN/NP with clinical experience and strong communication skills, able to work remotely and independently while meeting performance

Qualifications

• Active RN, BSN or MSN, NP in good standing.
• 1-3 years of clinical experience.
• Customer service experience in a health care setting.

Responsibilities

• Deliver exceptional service to members, providers and care team members through multiple channels.
• Review cases with external risk team and support care plans as needed.
• Conduct intake assessment, coordinate treatment plans and ensure high-value in-network care.
• Medication reconciliation and post-visit care coordination.
