Service Coordination Unlimited Inc. is seeking a compassionate Supports Coordinator to help individuals achieve person-centered goals and independent living in our Erie region.

The role requires travel within Erie County and surrounding areas, while offering work-from-home flexibility with required travel. The position emphasizes coordinating services, maintaining documentation, ensuring compliance, and providing participant-focused support to enable better outcomes.

Qualifications

• Bachelor’s degree with at least 12 eligible credits required (social work/psychology or related field preferred).
• Practicum experience is a plus.

Responsibilities

• Assess, identify, secure, and monitor services and supports for participants.
• Respond to participant needs and communicate across channels.
• Meet compliance standards and timeframes for assessments and plans.
• Document visits and contacts in required systems.
• Provide training and support to participants and consumer employees.
• Maintain internal databases and track participant information.
• Assist participants in obtaining habilitation, vocational, and other services.
• Demonstrate company values in all interactions.
