Job summary

Service Coordination Unlimited in Scranton, PA, is hiring a Service Coordinator I to help individuals access services and live independently. You will assess needs, arrange supports, and monitor plans while working remotely with occasional in-person visits.

This role requires a Bachelor’s degree or 3+ years in social services, excellent communication, and reliable transportation to travel across Lackawanna and Luzerne counties. A sign-on bonus is available to join our compassionate team.

Qualifications

• Bachelor’s degree required in social work, psychology, or related fields.
• At least three years in a social services or healthcare setting.
• Experience with people with disabilities or seniors and LTSS/Case Management preferred.
• Strong communication and teamwork skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess, identify, secure, and monitor services for program participants.
• Respond to participant needs via multiple communication methods.
• Meet compliance standards and timeframes for plans, visits, calls, and reports.
• Document visits and contacts in required systems.
• Provide training and support to participants and consumer employees.
• Maintain internal databases to track participant information.
• Assist participants with Accessibility Adaptations, equipment, and transportation.
• Uphold company values in all interactions.
