Job summary

United Disabilities Services in Lancaster, PA seeks a Service Coordinator to manage person-centered service plans for individuals with disabilities and older adults, coordinating needed services to improve independence and quality of life. The role is remote from home with travel across Lancaster and Berks counties in Pennsylvania.

The successful candidate will have a bachelor’s degree in social work, psychology, or related fields with practicum experience, or at least three years of case

Qualifications

• Bachelor's degree in social work, psychology or related field with practicum experience.
• Alternatively, at least three years of case management experience in social services or healthcare.

Responsibilities

• Develop a person-centered service plan addressing identified needs, goals, interventions and progress.
• Coordinate services for participants based on identified needs.
• Monitor authorized services through in-home visits, phone contacts, and planning meetings; liaise with service providers.
