Job summary

Thee Agency in the United States is seeking a compassionate Supports Broker to join our growing team. The Supports Broker will work alongside individuals receiving self-directed services through the New Jersey Division of Developmental Disabilities (DDD) to identify goals, navigate resources, and build meaningful plans that promote independence, choice, and community inclusion.

This remote position requires advocacy, relationship-building, and strong communication.

Qualifications

• Experience working with individuals with intellectual and developmental disabilities.
• Knowledge of NJ DDD services and self-directed supports.
• Strong communication, organization, and problem-solving skills.
• Ability to build trusting relationships with individuals and families.
• Willingness to travel throughout New Jersey.

Responsibilities

• Work directly with individuals and families to identify goals, preferences, and outcomes.
• Provide guidance and support related to self-directed services and resources.
• Assist with navigating community resources, including housing, transportation, employment, recreation.
• Support individuals through the housing search process and applications.
• Advocate for individuals while centering their choices.
• Maintain communication with individuals, families, support coordinators, providers, and others.
• Complete required documentation, progress notes, service logs, and reports.
• Assist with understanding and maximizing approved self-directed budgets.
• Attend meetings and collaborate with the support team.
