Job summary

Public Partnerships LLC seeks consultants (Support Brokers) to help individuals with disabilities and seniors live independently in California. You will enroll participants and their direct care workers, train on program rules, and develop spending plans for monthly budgets while monitoring health and safety.

Training will be provided and strong client service is essential. Role requires coordinating with participants, recruiting resources, responding to inquiries, and maintaining progress notes

Qualifications

• BA/BS in related field, or 4+ years of related experience substituted for degree.
• Strong oral and written communication skills.
• Ability to manage own schedule and work independently with minimal supervision.

Responsibilities

• Manage workload of up to 125 program participants monthly.
• Maintain required contact with participants via phone and email.
• Provide information and assistance to help participants exercise choice and control over services.
• Enroll participants and direct care workers; monitor health and safety; maintain progress notes.
• Coordinate with management and respond to participant inquiries.
