Job summary

Teladoc Health is seeking a Nurse Care Coordinator to provide proactive utilization management and care coordination for members with high-risk needs. You will collaborate with care teams and external providers to ensure high-quality, cost-effective care.

The role emphasizes postpartum/post-visit care navigation, eligibility screening for support services, and data-driven decision making, in a flexible remote environment. Strong communication and clinical assessment skills are essential.

Qualifications

• Active RN, BSN or MSN, NP in good standing.
• 1-3 years of clinical experience.
• Customer service experience in a health care setting.
• Ability to work independently and as part of a high performing team.
• Bilingual (English/Spanish) a plus.

Responsibilities

• Deliver exceptional service to members, providers, care team members and external facilities via multiple channels.
• Review members’ cases with external health plan risk team and provide advice, direction, and support.
• Meet with members and patient navigators after primary care physician appointments to review plans of care.
• Coordinate treatment plans and conduct re-assessment of services to ensure high-value, in-network care.
• Medication reconciliation.
