Alignment Health is seeking a bilingual Spanish care coordinator to join the remote Care Anywhere team. You will perform high-volume outbound outreach after hospital or SNF discharges, schedule follow-ups, and coordinate care resources.

This role emphasizes patient advocacy, documentation, and cross-team collaboration in a remote setting. Candidates should have 1+ year healthcare experience, a high school diploma or GED, and prefer TOC/post-discharge outreach experience.

Qualifications

• High School Diploma or GED is required.
• Completion of an accredited Medical Assistant program.
• Minimum 1 year of health care experience (health plan, medical office, IPA, MSO).
• Preference for experience with transitions of care (TOC) and post-discharge outreach.
• Bilingual English/Spanish preferred.

Responsibilities

• Conducts high-volume outbound telephonic outreach to members after hospital or SNF discharges.
• Follow time-sensitive outreach protocols and complete required outreach attempts and follow-ups.
• Assist with scheduling timely follow-up appointments with Care Anywhere providers.
• Capture required information using established scripts and questionnaires.
• Document member outreach, outcomes, and follow-ups in required systems.
