Qualifications

Responsibilities

• Conducts high-volume outbound telephonic outreach to members following Hospital and SNF discharges.
• Engages members to schedule timely follow-up appointments with Care Anywhere providers.
• Documents outreach outcomes and follow-up activities in required systems.
