Job summary

Alignment Health is seeking a compassionate, customer service oriented care coordinator to join the remote outpatient case management team. This fully remote role supports the RN case manager, coordinates services, closes care gaps, and helps members navigate their care plans.

In this position you will assist with scheduling, documentation, and outreach while maintaining confidentiality and ensuring timely, appropriate care for seniors and chronically ill members.

Qualifications

• Minimum 1 year experience in health care such as health plan, medical office, IPA, or MSO.
• Bilingual English and Spanish preferred.
• Able to read and write routine reports and correspondence.
• Proficient in Microsoft Office suite; basic math and data entry skills.

Responsibilities

• Reach out to members telephonically to assist with referrals, authorizations, HHC, DME needs, medication refills, and follow-ups.
• Create cases, tasks, and complete documentation in the case management module for hospital/SNF discharges.
• Comply with and document tasks assigned by nurse.
• Work with the case manager to engage and manage a panel of members in a fully remote role.
• Maintain confidentiality of information between health care professionals and members.
