Job summary

Devoted Health, Inc. is seeking a Social Work Case Manager to support the Complex Care Department, coordinating services and care plans for high-need patients from a remote setting in the United States.

You will collaborate with clinicians and care teams, identify social barriers, provide caregiver support, and facilitate advanced care planning when needed. Requires 3+ years in medical social work and MSW license; Spanish bilingual preferred.

Qualifications

• Minimum of 3 years working in medical social work, case management, or care coordination.
• Master’s degree in social work and active state license required.
• Bilingual in Spanish preferred; health-insurance experience a plus.

Responsibilities

• Establish trust with patients telephonically and virtually.
• Collaborate with interdisciplinary team to ensure patient-centered care.
• Identify social barriers and coordinate resources to address psychosocial needs.
• Support care planning and end-of-life discussions when appropriate.
• Coordinate care between behavioral health and medical teams and monitor outcomes.
