Job summary

Mercy Health is seeking a Social Worker to provide remote clinical care management for eligible patients, coordinating care to improve outcomes and reduce costs. The role requires working with an Interdisciplinary Team and familiarity with Hampton Roads, VA resources.

Responsibilities include managing patient caseloads, developing care plans, coordinating with PCPs and specialists, and documenting in the EMR. 2–3 years of relevant experience and appropriate licensure are expected.

Qualifications

• Bachelor's degree is required.
• 2-3 years acute care, home health or case management experience.
• Licensure as required by state of practice; Case Management certification preferred.
• LSW or LCSW preferred.

Responsibilities

• Maintain a caseload according to department policies.
• Identify, enroll and manage patients in Complex Case Management.
• Develop and implement care plans with periodic reviews.
• Collaborate with IDT members to implement patient-centered plans.
• Assess social determinants of care and set goals in the care plan.
• Document communications in the EMR and coordinate referrals to resources.
• Assist with advanced care planning and directing to community resources.
