Job summary

Alliance Health is seeking a Complex Case Manager to support members with high complexity, including multiple chronic conditions and SMI/SUD. The role is remote with NC-based duties, requiring occasional onsite meetings in Charlotte, NC, and travel across Mecklenburg County to engage with members and providers.

Responsibilities include initial engagement, care planning, coordination, and documentation in Alliance’s system, with emphasis on evidence-based, person-centered care and optimizing

Qualifications

• RN licensure with two years post-degree care management/coordination experience in Behavioral Health and Physical Health conditions.
• Master’s in Human Services with two years in care management; active licensure as LCSW/LMFT/LCAS/LCMHC/LPA preferred.
• Bachelor’s in Nursing with two years care management/coordination experience in Behavioral Health and/or Complex Care Management.

Responsibilities

• Engage members with high complexity to address health and care coordination needs.
• Inform members about eligibility for the program and how to use services.
• Schedule assessments and develop individualized care plans with ongoing coordination.
• Coordinate transitions of care and link to community resources.
• Document engagements in Alliance’s electronic care-management system.
• Travel between Alliance offices and meet members, providers, and stakeholders as needed.
• Ensure documentation meets state, agency, and Medicaid requirements.
