Job summary

Devoted Health is seeking a Manager of Social Work Case Management to build, lead, and inspire a remote team of Social Worker Case Managers and behavioral health care coordinators, partnering with cross-functional teams to standardize workflows and improve patient outcomes.

You will spend 30-40% on direct patient outreach and case work, drive onboarding for new teammates, and develop plans of care that align community, clinical, and financial resources.

Qualifications

• Licensed social worker (LMSW or higher) required.
• 3+ years in case management.
• 3+ years in people management, developing and retaining a high-performing team.
• Experience with interdisciplinary care management teams.

Responsibilities

• Build and develop a high-performing team of Social Worker Case Managers and behavioral health care coordinators.
• Track staff performance through competency check-offs, audits, shadowing, and 1:1s, and coach where needed.
• Create workflows, assessments, and plans of care to build efficiency and measurable patient outcomes.
• Support the team on escalated patient needs.
• Lead onboarding for new teammates and ongoing education for existing staff.
• Partner with cross-functional teams to drive execution and standardize workflows across programs.
• Meet with the Senior Manager of RN and SW TOC functions to review performance and improvements.
• Spend 30-40% of time on direct patient outreach and case work.
