Job summary

Alliance Health is seeking a Complex Case Manager for a remote role based in North Carolina. You will engage with members with high complexity and coordinate across providers, ensuring timely assessments and care plans.

The role requires RN licensure with 2 years of relevant experience or a Master’s in Human Services with similar experience. Travel to regional counties may be required. Hybrid/remote options are available.

Qualifications

• RN licensure with 2 years of post-degree care management or coordination experience.
• Master’s degree in Human Services or related field with 2 years of relevant experience; full licensure may be required.
• Care management certification preferred.

Responsibilities

• Engage with members and providers to address health and care coordination needs.
• Develop individualized care plans and provide coordination and follow-up.
• Perform assessments, planning, and evidence-based interventions throughout the care continuum.
• Promote medication safety and address social determinants of health.
• Document all engagements in Alliance’s care-management system.
