Job summary

Community Health Plan of Washington is seeking a Remote Case Manager to manage case delivery and coordination programs. Successful candidates will have a strong nursing or social work background, managing members' healthcare needs while promoting their quality of life.

The role requires collaboration with various healthcare teams and knowledge of care management best practices. Candidates should possess strong organizational skills and be comfortable working independently.

Qualifications

• Minimum three (3) years of clinical experience in an acute care environment.
• At least one (1) year case management experience or equivalent.
• Knowledge of Medicaid and Medicare regulations.

Responsibilities

• Perform telephonic case management for members with acute and chronic needs.
• Advocate for members and facilitate resource coordination.
• Assess and document care of members in the system.

Skills

Bilingual abilities

Proficiency with Microsoft Office products

Managed care (Medicaid and/or Medicare) experience

Education

Bachelor’s degree in nursing or Master’s in social work

Current license in Washington as RN or LSWAA
