Job summary

Devoted Medical Group seeks a Social Work Case Manager to support the Complex Care Department. You will coordinate services for patients with complex chronic conditions, frail elders, and high-risk individuals, collaborating with clinicians, case managers, and community health workers to develop tailored care plans.

Requires a Master’s in Social Work and active state licensure, with 3+ years in medical social work; Spanish language skills preferred.

Qualifications

• Master’s degree in social work and active state license.
• Minimum 3 years in medical social work, case management or care coordination.
• Bilingual in Spanish preferred.

Responsibilities

• Establish trust with patients telephonically and virtually.
• Collaborate with an interdisciplinary team to provide patient-centered care.
• Identify social barriers and coordinate services for complex patients.
• Conduct screenings and create personalized action plans.
• Navigate family and support issues and provide caregiver resources.
• Discuss end-of-life planning when appropriate.
