• Experience using InterRAI and PCSP tools for annual assessments.
• Strong interpersonal and written communication skills.
• Ability to coordinate care with multidisciplinary teams.

Responsibilities

• Conduct comprehensive person-centered assessments using InterRAI/PCSP.
• Develop individualized service plans with participants and families.
• Identify and coordinate LTSS and community resources.
• Facilitate provider and SC choice and adherence to timelines.
• Complete documentation and reporting per MCO standards.
• Attend interdisciplinary team meetings and conduct home visits.
