Job summary

PCA Care Connections is seeking a Service Coordinator Trainee to facilitate and coordinate long-term supports, working primarily remotely with office access as needed.

You will conduct assessments, develop individualized service plans, and coordinate with providers and families to meet participants’ needs and goals. Strong communication, organization, and cultural competency are essential.

Qualifications

• Education: High School Diploma or equivalent.
• Experience: Minimum of three years in a social service or health care related setting.
• Professional characteristics: energy, communication, organization, flexibility, critical thinking, cultural competency.

Responsibilities

• a. Conducts comprehensive person-centered assessments utilizing InterRAI and PCSP tools.
• b. Develops individualized service plans with participant, family, and caregivers.
• c. Identifies and arranges LTSS and related services.
• d. Facilitates participant choice of providers.
• e. Adheres to all MCO timeframes.
• f. Documents and reports as required.
• g. Participates in Interdisciplinary Team meetings.
• h. Conducts home visits and calls to monitor plan progress.
• i. Facilitates care transitions.
• j. Coordinates with behavioral health services.
• k. Monitors participant satisfaction.
• l. Supports CHC eligibility maintenance.
• m. Completes incident reports and RCA as needed.
• n. Notifies rights, assists with complaints and fair hearings
• o. Ensures HIPAA compliance.
