Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field required.
• Minimum of three (3) years of experience in service coordination, case management, or related human services role preferred.
• Experience with waiver-funded or I/DD populations strongly preferred.

Responsibilities

• Coordinate services across Medicaid Waiver programs, MA State Plan, and other resources.
• Maintain participant-centered planning, service coordination, compliance, and documentation.
• Conduct assessments and develop individualized service plans at intervals stated in policy.
• Enter required documentation accurately and within established timelines.
• Coordinate with healthcare providers, community agencies, and government entities.
• Advocate for participant needs and promote independence and quality of life.
