Job summary

Service Access & Management, Inc. (SAM) seeks a Service Coordinator Supervisor to lead a Long Term Support and Services unit under Pennsylvania’s Community HealthChoices program.

You will guide a team of coordinators, oversee care navigation, and promote independent, person-centered planning in a high-accountability environment. This role is based in Berks County with remote work options and a Monday–Friday schedule.

Qualifications

• RN license, or PA-licensed social worker, or licensed mental health professional.
• At least three (3) years of relevant experience.
• Supervisory, leadership, or management experience preferred.
• Ability to manage time effectively and respond to changing priorities.

Responsibilities

• Ensure that service plans are developed and implemented.
• Ensure monitoring and follow-up are conducted appropriately.
• Verify waiver eligibility and Medical Assistance requirements.
• Support employees in meeting documentation and billing standards.
• Monitor unit productivity and provide coaching as needed.
• Oversee the incident management and reporting process.
• Ensure timely and accurate completion of assessments and planning documents.
• Provide direct supervision and caseload support when necessary.
• Complete required trainings and prepare program reports.
• Maintain cooperative and professional relationships with colleagues and stakeholders.
