Job summary

Humana Inc. seeks a Senior Specialty Consultative Pharmacist to assess and manage member medication therapy via telephonic interactions. You will develop care plans, monitor progress, educate patients, and collaborate with a team in a remote environment.

You will work with inbound/outbound calls, review prescriptions for accuracy, and resolve drug interactions. Strong clinical expertise is required along with multi-system navigation skills for remote work.

Qualifications

• Pharmacy degree from an accredited college (PharmD).
• Active Pharmacist license in Florida, Texas, Ohio or Arizona.
• Minimum three years of experience as a pharmacist in a high-volume setting.
• Proficient with Microsoft Office applications and multiple systems.

Responsibilities

• Perform inbound and outbound calls to discuss medication therapy with members.
• Assess and evaluate medication therapy and disease state needs.
• Develop and implement comprehensive care plans for patients.
• Monitor progress through regular assessments and education via telecommunication.
• Identify drug interactions and adverse events and report appropriately.
