Job summary

Sagility is seeking experienced Registered Nurses (RNs) to join our team as Senior Care Specialists in a work-from-home capacity. This seasonal opportunity has a tentative end date of June 2027, with potential bonuses and incentives based on performance and business needs.

As a Senior Care Specialist, you will assess members by phone, develop individualized care plans, provide education, and coordinate services to improve health outcomes.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and in a remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple electronic medical records systems.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME and home health services.
• Help members navigate healthcare benefits, services, and options.
• Assess physical, environmental, and psychosocial factors affecting health and wellbeing.
• Collaborate with multidisciplinary teams including social workers, dietitians, pharmacists, and providers.
• Promote member self-management with education and resources.
• Coordinate care and condition-management activities among members and care team.
• Document member interactions and outcomes in applicable systems.
