Job summary

Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity. This seasonal position runs through June 2027 with potential bonuses and incentives based on performance and business needs.

You will assess members by phone, develop care plans, educate and coach on chronic conditions, and coordinate services to improve outcomes. Strong nursing judgment and remote-work ability are essential.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license allowing practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple EMR systems.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic conditions.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Navigate healthcare benefits and available care options with members and caregivers.
• Document interactions, assessments, care plans, interventions, and outcomes accurately in systems.
