Call 4 Health is seeking an experienced Registered Nurse to join our 24-hour remote triage team. In this role, you will deliver clinical assessments via telephone and patient portal, document in EHRs, and support providers across multiple states from a home office.

The ideal candidate has an active compact license, 2–4 years RN experience (ER/urgent care or ICU preferred), strong triage skills, and the ability to work in a fast-paced, collaborative environment with flexible scheduling.

Qualifications

• RN degree (Associates or BSN) required.
• 2–4 years of full-time RN clinical experience, preferably ER/Urget Care, Adult, Pediatric, Ambulatory Care, or ICU.
• Experience with telephone triage and electronic triage systems.
• Experience using two or more EHR platforms.
• Active, unencumbered e-NCL/Enhanced Nurse Licensure Compact license.
• Willingness to obtain additional state licenses as requested.

Responsabilités

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal.
• Support providers with administrative and clinical tasks, including medication refills and notifications of critical results.
• Coordinate post-discharge and post-operative follow-up and referrals.
• Collaborate with hospitals, nursing homes, funeral homes, and government agencies as needed.
• Participate in Remote Patient Monitoring programs (vital signs, chronic disease education).
• Screen and qualify patients for clinical trials and educate patients.
• Take on additional responsibilities to support team success and patient outcomes.
