Job summary

Call 4 Health is seeking an experienced Registered Nurse (RN) for a Remote Triage role. You’ll provide clinical assessments and triage over the phone and patient portal, using established protocols to support patients and providers nationwide.

Ideal candidates hold an active nursing license (e-NCL/ECNL) and have 2–4 years of RN experience, including telephone triage. This remote position requires strong communication and documentation skills.

Qualifications

• RN with an active license (e-NCL/ECNL) and multi-state readiness.
• 2–4 years of full-time RN clinical experience, preferred ER/Urgent Care.
• Experience with telephone triage using structured protocols.
• Willingness to obtain additional state licenses as needed.

Responsibilities

• Respond to inbound/outbound calls for multi-state nurse triage service.
• Perform clinical assessments and triage per Schmitt-Thompson protocols.
• Document interactions accurately in client EHR systems.
• Support providers with referrals, prior authorizations, and care coordination.
• Collaborate with hospitals, nursing facilities, and agencies as needed.
• Participate in Remote Patient Monitoring and patient education.
• Screen patients for clinical trials and provide education.
• Adapt to schedule changes and contribute to team success.
