Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care beyond clinic hours via telephonic support. You will address medical, social, and psychological needs, providing confident patient service while documenting interactions accurately.

The role requires an active RN license across OSH practice states, remote work capability, and strong time-management with reliable attendance. Weekly hours are 40, with a base pay range reflecting experience and geography.

Qualifications

• Active RN license (non-probationary) in all OSH practice states.
• Ability to manage time and work autonomously with reliability.
• Strong verbal and written communication skills for patient interactions.

Responsibilities

• Provide confident, professional service to OSH patients.
• Follow current Prescription Refill policy.
• Maintain focused engagement with patients during calls.
• Document clearly and completely during and after calls.
• Complete documentation for all patient requests before ending a call.
• Check CareReq queue and ensure CareReqs are complete.
• Schedule appointments and arrange transportation for clinic visits.
• Support departmental metrics and goals.
• Perform other duties as assigned.
