Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care beyond clinic hours by addressing patients’ medical, social, and psychological needs telephonically. You will provide calm, professional service and ensure accurate documentation while supporting continuity of care.

The role requires an active RN license in all OSH practice states, strong time management, reliability, and the ability to work flexibly including evenings and weekends.

Qualifications

• Active, non-probationary RN license in all OSH practice states (US).
• Ability to manage time and work autonomously with reliability.
• High level of conscientiousness and reliability on calls.
• Flexible to work evenings, weekends, and holidays.
• Proactive maintenance of licenses and CE requirements.

Responsibilities

• Provide professional service to Oak Street Health patients over the phone.
• Follow current Prescription Refill policy.
• Document clearly during and after each call.
• Schedule appointments for patients and arrange transportation as needed.
• Meet department metrics and complete all tasks within role scope.
• Coordinate with remote clinic operations as required.
