Job summary

Oak Street Health seeks a Clinical Call Center Triage Nurse to extend care beyond clinic hours by addressing patients' medical, social and psychological needs over the phone. You will provide confident service, document thoroughly, and help schedule appointments and transportation for patients.

Progression from Triage RN I to II is structured, with emphasis on reliability, learning, and performance metrics. This is a full-time remote role with benefits and ongoing remote training.

Qualifications

• Active, non-probationary RN license in all OSH practice states.
• US work authorization and remote work experience are required.
• Ability to manage time autonomously with reliability.
• Flexibility to work evenings, weekends or holidays as needed.

Responsibilities

• Provide confident, professional service to OSH patients.
• Follow current Prescription Refill policy.
• Document clearly during calls and complete requests before ending calls.
• Schedule appointments and arrange transportation for patients.
• Support CareReq processes and meet departmental metrics.
• Perform other duties as assigned.
