Lehigh Valley Health Network is seeking an experienced RN to provide triage telephone service and telephonic assessment, guiding patients to the appropriate level of care. You will support care coordination, validate symptoms, and document interactions in patient records from a remote Pennsylvania location.

The role requires an Associate’s Degree in Nursing, PA RN license, and 5 years of nursing experience with strong computer skills and patient empathy.

Qualifications

• Associate’s Degree in Nursing is required.
• 5 years nursing experience across acute care, ED, medical office, or telephone triage.
• 2 years of clinical acute care hospital experience/urgent care.
• RN license in Pennsylvania.
• Strong computer skills and patient empathy.

Responsabilités

• Directs callers to appropriate level of care or provides healthcare advice using approved guidelines.
• Maintains medical records and files patient charts accurately.
• Provides clinical support in symptom assessment and patient education.
• Coordinates care across settings; ensures labs and referrals are completed and discussed.
• Performs quick triage to assess illness severity and document findings.
• Communicates with patients, physicians, and hospital staff as a liaison.
• Refills prescriptions and orders for testing or labs as needed.
