Qualifications

• Associate's Degree in Nursing.
• 5 years experience in nursing (acute care, ED, medical office, telephone triage)
• 2 years previous clinical acute care hospital experience/urgent care.
• Excellent computer skills and empathetic patient service.
• Knowledge of telephone-based clinical assessment techniques and triage protocols.
• RN Licensed in Pennsylvania.

Responsibilities

• Directs callers to appropriate level of care and provides healthcare education using guidelines and protocols.
• Maintains medical records and files patient charts/reports.
• Provides clinical support in symptom assessment and patient education.
• Coordinates care across settings for a designated population; ensures labs and referrals are completed.
• Performs a brief evaluation to estimate illness/injury severity and documentation.
• Serves as a liaison between patients, physicians, and hospital staff.
• Refills prescriptions and requests tests as appropriate.
