Job summary

Intermountain HealthCare Inc. is seeking an RN Transition and Triage Care Manager. The role provides telephonic/digital care coordination across settings, ensuring continuity and appropriate level of care for patients and families.

Responsibilities include proactive assessment, care planning, remote triage, and education. Remote work is supported; location is Las Vegas with periodic local references as needed.

Qualifications

• Current RN license in state of practice.
• BSN required within four years of hire.
• Chronic disease management experience.
• Understanding of disease management and barriers to care.
• Basic computer skills and MS Office proficiency.

Responsibilities

• Identify patients for proactive interventions using screening criteria and assess medical, functional, and social needs.
• Coordinate with services for social determinants of health and community care.
• Perform remote nursing triage and evidence-based recommendations.
• Support self-management, care navigation, and patient education to address barriers.
• Educate care teams on transitions/triage and advocate for patient needs.
• Collaborate with physicians and other care team members to ensure timely, quality care.
