Qualifications

• Current, unrestricted Compact RN license in Arizona.
• 5+ years of recent experience in the acute inpatient hospital setting.
• 2+ years of discharge planning and/or case management.
• Experience with Medicaid/Medicare/Commercial insurance products and related regulatory requirements.
• Proficient in MS Word, MS Excel, MS PowerPoint and MS Outlook.
• Permanent residence within the state of Arizona.

Responsibilities

• Evaluate discharge needs, delays in care, and readmission prevention.
• Coordinate post-discharge care with providers and members.
• Participate in rounds with Medical Director as needed.
• Identify internal or community-based program support or resources.
• Coordinate with Discharge Planner to arrange post-hospital services.
• Assist with placement in alternate level of care facilities when needed.
• Document discharge activities per SOPs and data-entry standards.
• Participate in team meetings and education discussions.
