Job summary

Intermountain Health in Denver, CO seeks an RN Transition and Triage Care Manager to deliver time‑limited care management across settings, coordinating care and guiding patients to the appropriate level of care by phone or digital means.

You will identify patients, develop care plans, and collaborate with clinicians, social services, and post‑acute providers to improve transitions and reduce readmissions. Requires RN license, BSN within 4 years, and experience in chronic disease management.

Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution; must obtain BSN within four years of hire.
• Demonstrated clinical nursing experience in chronic disease management.
• Familiarity with chronic disease terminology and processes.
• Understanding of disease management including treatment, LOS, barriers to care.
• Basic computer skills and knowledge of Microsoft Office.

Responsibilities

• Identify patients for proactive interventions using screening criteria and referrals; develop individualized care plans.
• Coordinate with internal and external services for social determinants of health needs; evaluate outcomes.
• Conduct remote nursing assessments and triage using evidence-based protocols.
• Support patient self-management and education to address barriers.
• Educate healthcare team about transitions and referrals; advocate for patient rights.
• Collaborate with physicians and care team to ensure timely, quality care; participate in rounds or case conferences.
• Build partnerships with hospital care management and post-acute providers to ensure continuity of care.
• Participate in system and regional initiatives to improve transitions of care.
• Perform data analysis on post-acute stays, readmissions, and trends affecting ambulatory/post-acute services.
• Promote Intermountain Health values in care delivery.
