Job summary

UnityPoint Health is seeking a Remote RN Care Coordinator to support safe transitions from hospital to home through telephonic care coordination and patient education for high-risk patients.

Qualifications

• Active RN license and at least 1 year of clinical experience.
• Home care or ambulatory care experience preferred; EPIC familiarity a plus.

Responsibilities

• Conduct post-discharge outreach and assessments in Epic work queues.
• Coordinate services, medication reconciliation, and community resources.
• Collaborate with providers and adapt to weekend/holiday rotation.
