Job summary

Intermountain Health is seeking an RN Transition and Triage Care Manager to provide time-limited, patient-centered care coordination across settings, primarily from a remote environment. The role emphasizes proactive assessment, care planning, and collaboration with providers, payers, and families to improve transitions and outcomes.

The position supports a Monday–Friday schedule with shifts that may include 5/8 or 4/10 patterns.

Qualifications

• Current RN license in state of practice.
• BSN required or must obtain BSN within four years of hire or promotion date.
• Demonstrated clinical nursing experience in chronic disease management.
• Demonstrated understanding of disease management including treatment and barriers to delivery of care.
• Basic computer skills and knowledge of Microsoft Office software.

Responsibilities

• Identify patients for proactive interventions and develop individualized care plans.
• Coordinate with internal/external services for social determinants of health needs and community care.
• Conduct remote nursing assessments using evidence-based protocols.
• Support patient self-management through health coaching and care navigation.
• Educate care team and patients about transitions and triage processes.
• Collaborate with physicians and healthcare team to ensure timely, quality care.
• Participate in rounds or case conferences as needed.
• Analyze data to identify trends in extended post-acute stays and readmissions.
