Job summary

Alpine Physicians is seeking a Care Manager (RN) to deliver clinically and psychosocially informed, person-centered care management for Medicaid members with complex needs. You will assess, plan, and coordinate care, manage transitions, and collaborate with interdisciplinary teams to improve outcomes.

The role requires outbound outreach to engage hard-to-reach members, adherence to productivity standards, and HIPAA-compliant documentation. Remote options may be available.

Qualifications

• Active unrestricted RN license in the state you serve.
• 1+ year(s) of experience in care management or related field.
• Experience with high-risk, medically complex, or psychosocially complex populations.
• Experience supporting transitions of care.

Responsibilities

• Perform comprehensive clinical and psychosocial assessments for high‑risk, medically complex members.
• Develop, implement, and update individualized care plans addressing medical, behavioral health, psychosocial, and environmental needs.
• Manage transitions of care after hospitalization or acute episodes.
• Conduct follow-up for transition-of-care within established timelines.
• Coordinate care with PCPs, specialists, and community resources.
• Provide education to patients and caregivers on disease management and care navigation.
