Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution; must obtain BSN within four years if not already held.
• Minimum of 2 years nursing experience; ambulatory and telephonic triage experience preferred.
• Basic computer skills and knowledge of Microsoft Office.

Responsibilities

• Identify patients for proactive interventions and develop individualized care plans.
• Coordinate services for social determinants of health and community care.
• Conduct remote nursing assessments and triage to determine care needs.
• Support patient self-management through health coaching and navigation.
• Educate patients and families about conditions and self-management.
• Collaborate with care team and community resources to ensure timely care.
• Develop partnerships with hospital and post-acute providers for seamless transitions.
• Participate in process improvements to reduce duplicative services.
• Analyze data to track outcomes and monitor trends affecting care.
