Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution; BSN must be obtained within four years.
• Chronic disease management experience and familiarity with disease management terminology.
• Proficient with basic computer skills and Microsoft Office.
• Minimum 2 years of nursing experience; no new grads.

Responsibilities

• Identify patients for proactive interventions and develop individualized care plans.
• Coordinate with services for social determinants of health needs.
• Conduct remote nursing assessments and use evidence-based protocols.
• Support patient self-management through health coaching and care navigation.
• Educate care teams and patients about transitions, referrals, and self-management.
• Collaborate with physicians and other team members to ensure timely care.
• Develop partnerships with hospital and post-acute providers to ensure seamless transitions.
• Participate in system initiatives to improve transitions of care.
