Job summary

Qualifications

• RN license in state of practice.
• BSN within four years of hire date for RNs.
• 2+ years of nursing experience; chronic disease management experience preferred.

Responsibilities

• Identify patients for proactive interventions using screening criteria and referrals.
• Coordinate care with social determinants of health (SDoH) services and community resources.
• Conduct remote nursing assessments and apply evidence-based protocols to guide care.
• Educate patients and families on self-management and navigate care pathways.
• Collaborate with physicians and care team to ensure timely, quality care and address delays.
