Molina Healthcare is seeking a remote RN Care Manager. The role prefers candidates in Washington State and aligns with Pacific Standard Time hours. You will support Medicare members via telephonic case management and transition-of-care tasks, with no nighttime shifts or patient-facing care.

Qualificações

• Active RN license in state of practice.
• 2+ years healthcare experience, care management preferred.
• Valid driver’s license and reliable transportation for travel.
• Knowledge of EMR systems and HIPAA regulations.
• Strong communication and interdisciplinary teamwork skills.

Responsabilidades

• Coordinate care and collaborate with multidisciplinary teams to optimize member outcomes.
• Complete comprehensive assessments and determine eligibility for care management.
• Develop and implement care plans with members and providers.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and adjust care plans; document interventions and outcomes.
• Maintain ongoing member caseload and facilitate ICT meetings.
• Educate and support members using motivational interviewing.
• Identify barriers to care and coordinate resources accordingly.
• May provide medication reconciliation and peer consultations.
• Possible 25–40% local travel based on state/contractual needs.
