Job summary

Call 4 Health is seeking an experienced Registered Nurse (RN) for a Remote Triage role. You will support patients and providers nationwide via telephone triage, EHR management, and clinical coordination from a home office.

The ideal candidate holds an active nursing license (e-NCL/Compact) and has 2–4 years of full-time RN experience, including telehealth or triage experience, with proficiency across multiple EHR systems.

Qualifications

• RN license active and unencumbered (e-NCL/Enhanced Nurse Licensure Compact).
• 2–4 years of full-time RN clinical experience, preferably ER/urgent care, adult, pediatric, ambulatory care or ICU.
• Experience with two or more EHR platforms.

Responsibilities

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize triage protocols to assess and triage patients by phone or portal.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal per established protocols.
• Support providers with administrative and clinical tasks, including medication refills and post-discharge follow-up.
• Facilitate referrals and assist with prior authorization requests.
• Collaborate with hospitals, nursing homes, and government agencies as needed.
• Participate in Remote Patient Monitoring programs, including vital signs monitoring and chronic disease education.
• Screen and qualify patients for clinical trials and provide patient education.
