Job summary

Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a remote, work-from-home capacity. This seasonal role runs through June 2027 with potential bonuses and incentives based on performance.

You will assess members by phone, develop care plans, provide education, and coordinate services to support health goals for Medicare/Medicaid populations in a Whole Person Care approach.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple computer systems and electronic medical records.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Coordinate care and interventions for acute situations using clinical nursing judgment.
• Support transitions of care and arrange DME, home health, and provider appointments.
• Assist members in navigating benefits and available care options.
