Job summary

Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity. This seasonal role runs through June 2027 with potential bonuses based on performance.

You will assess members via phone, develop care plans, provide education, and coordinate services, using a Whole Person Care approach in collaboration with care teams. Remote RN work requires strong judgment, ability to work independently, and proficiency with EMR systems and common office

Qualifications

• Active, unrestricted RN license in state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency (MS Word, Excel, Outlook).
• Ability to navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess needs and coordinate interventions for acute situations.
• Coordinate care transitions, including DME, home health, and provider appointments.
• Navigate healthcare benefits and available care options with members and caregivers.
• Assess environmental and psychosocial factors affecting health.
• Collaborate with multidisciplinary teams to promote health outcomes.
• Document interactions, assessments, and outcomes accurately in EMR.
