Job summary

Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a remote, work-from-home capacity. This seasonal assignment ends around June 2027 and may include bonuses based on performance and company needs.

You'll assess and support members with chronic conditions by phone, developing personalized care plans and coordinating care with a multidisciplinary team to improve health outcomes.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience (3+ preferred).
• Strong clinical judgment, critical thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple systems and EMR.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and group health plan members.
• Develop actionable, individualized, and measurable care plans.
• Establish health coaching relationships and provide education on chronic condition management.
• Assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and discharge planning, including DME and home health services.
• Navigate healthcare benefits and available care options for members and caregivers.
• Collaborate with multidisciplinary teams to promote health outcomes.
• Document interactions, assessments, and outcomes accurately in systems.
