Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity. This seasonal position offers a tentative end date in June 2027, with bonus/incentive programs contingent on performance and business needs.

You will assess members by phone, develop care plans, provide education, and coordinate services to improve health outcomes for Medicare, Medicaid, and group health plan members.

Qualifikationen

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license allowing practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Aufgaben

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate health benefits, services, and care options.
• Assess factors affecting health and well-being; collaborate with care teams.
