Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) to work from home on a seasonal basis. This remote role supports members across the United States.

You will conduct telephonic health assessments, develop actionable care plans, coordinate transitions of care, and document outcomes to support safe, effective member care. Strong clinical judgment and remote work capability are required.

Qualifications

• Active, unrestricted RN license in home state.
• Compact RN license allowing practice in other states.
• Minimum 2 years bedside nursing experience; 3+ preferred.
• Ability to work independently in a remote environment.
• Strong communication and computer proficiency.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Coordinate care transitions and document activities accurately in systems.
