Job summary

Fairview Health Services is seeking an RN Navigator Oncology for a remote, casual position. The role involves coordinating care from referral to initial oncology consultation, educating patients and families, and collaborating with the multidisciplinary team to establish the initial care plan.

Ideal candidates will have an RN license in Minnesota, 3 years of adult oncology nursing experience, and be on track to obtain an Oncology Certified Nurse within 18 months.

Qualifications

• RN with oncology experience preferred.
• MN RN license required.
• Oncology Certified Nurse within 1.5 years preferred.

Responsibilities

• Navigate patient care from referral to initial oncology consultation.
• Serve as primary contact for patients new to cancer care.
• Coordinate initial care plan with multidisciplinary team.
• Educate patients and families about testing and next steps.
• Document and update EHR with initial treatment plan and care milestones.
• Collaborate with scheduling to arrange initial appointments.
