Job summary

Qualifications

• Bachelor of Science in Nursing required with RN licensure (RI)
• At least 3 years of nursing experience, including 2 in oncology
• Preferred Oncology certifications (OCN, AOCN, AOCNS) or CCM within 2 years of hire
• Comfort with using Slack, Zoom, Google Suite and EHRs

Responsibilities

• Provide direct patient care via phone and video.
• Deliver comprehensive care navigation from screening to survivorship or end-of-life care.
• Triage and respond to urgent clinical escalations.
• Identify and address patient needs including care coordination and referrals.
• Educate patients on diagnosis, Daymark Health’s care offerings and next steps.
• Build rapport with patients and care networks through empathetic communication.
• Collaborate with clinical team to ensure adherence to protocols and effective use of systems.
