Jobgether is seeking a Nurse Advisor (RN) based in the United States for a fully remote role. You will guide patients and families through healthcare options, coordinate care, and connect them with providers and services.

The position emphasizes compassionate communication, clinical assessment, and cross-functional collaboration. You will triage new clients, educate them about advisory services, and work with multidisciplinary teams to ensure timely and effective care pathways.

Qualifications

• Active RN license in the state of residence.
• Minimum 5 years in a clinical or patient-facing setting.
• Experience in case management, care coordination, or patient navigation.

Responsibilities

• Manage a portfolio of low- to moderate-acuity clients with clinical support and care coordination.
• Build strong relationships with patients and families as a trusted contact.
• Conduct clinical intake assessments via phone and email to determine needs and next steps.
• Help clients access providers, facilities, and services and coordinate appointments.
• Collaborate with client services coordinators and provider network teams for seamless care.
